Step 1: Request for Permission to Caption from the Publisher
This page is intended for Madison College Employees who wish to obtain permission to caption/convert copyrighted media. Please fill out and send the standard permission letter format (PDF, 256 KB) to the publisher of the media to be captioned/converted.
The permissions letter was created for use with Microsoft Word 97 - 2004. Please open the file in Microsoft Word and not in your browser for best functionality.
Please fill out all of the fields marked in yellow. Then print out and mail or email the letter to the publisher.

When you receive the written permission back from the publisher, please email a copy of that letter to the Helpdesk[at]madisoncollege [dot] edu (subject: Request_Permisions_Caption_Letter) (Help Desk). Your media will not be captioned/converted until written permission is on file with the Help Desk.
