Course Change Request Form

Welcome to the course change request form. The purpose of this web-based form is to ensure that the integrity of historical record keeping is maintained in a central database and to effectively comply with state reporting requirements. It is a tool that provides deans and center staff efficiencies in tracking, organizing and responding to your needs as quickly as possible.

SECTION 1: REQUESTOR/PROGRAM IDENTIFICATION
A copy of your submission is emailed to the email address listed.
An email notification is sent to the Associate Dean of the School selected in this section.
An email notification is sent to the Administrative Coordinator of the School selected in this section.
 
SECTION 2: CURRENT COURSE INFORMATION
The course catalog number is 8 digits. This field only accepts an 8 digit number, so do not use dashes when entering the number. For example, the 8 digit course catalog number for Intro to Sociology is 20809203.
SECTION 3: CHANGES BEING REQUESTED
