Welcome to the new course proposal request form. The purpose of this web-based form is to ensure that the integrity of historical record keeping is maintained in a central database and to effectively comply with state reporting requirements. It is a tool that provides deans and center staff efficiencies in tracking, organizing and responding to your needs as quickly as possible.

SECTION 1: REQUESTOR/PROGRAM IDENTIFICATION
 
A copy of your submission is sent to the email address listed.
An email notification is sent to the Associate Dean of the School selected in this section.
An email notification is sent to the Administrative Coordinator of the School selected in this section.
SECTION 2: COURSE INFORMATION
All program courses are limited to 5 or less credits. See the "Tool" at the top of the page for help on calculating these hours.
If required for a program or certificate, please identify which one(s) OR if used in the Liberal Arts Transfer program, please identify which requirement it satisfies: (Note: Make sure that you additionally input these where appropriate on the program revision form.)
You may add multiple Topics in this section provided the following criteria are met: 1) the only change you are requesting is to add Topics; 2) all the Topics are part of the same course catalog number listed in section 2 of this form; 3) you clearly identify each Topic Title; 4) the Topic description is less than 211 characters
The first two digits of the eight digit course number will reflect the program level of the course. The following program levels have been identified for use within the system: Aid code 10: General Education Aid code 20: Associate Degree, Liberal Arts Transfer Aid code 30: Technical Diploma (less than one year/2-25 credits) Aid code 31: Technical Diploma (1 year/26-54 credits) Aid code 32: Technical Diploma (2 year/55-70 credits) Aid code 50: Apprenticeship Aid code 47: Adult occupational/continuing education (offerings with a definite vocational/technical objective designed to provide future employment or upgrade individuals in their present occupations) Aid code 60: Adult avocational (offerings that are leisure-time, self-enrichment activities) Aid code 77: Alternative Learning
The subject area is a three digit number. Contact your admin coordinator for the subject area of your proposed course.
COMPASS cut scores must be approved by the MAAP committee. COMPASS scores are found on our website at http://madisoncollege.edu/compass-information. Contact the Director of Testing at 246-6330 if the requisites to this course differ from the approved scores provided by the MAAP committee.
