Nursing Assistant Registration Requirements

ALL Registration Requirements listed below must be completed and approved prior to enrolling in a Nursing Assistant course; no exceptions will be made.

1. Create a Madison College Student Account and Activate Account

2. English Reading Comprehension Requirement met by one of the following options:

  •  Assessment Testing (scores for one of the following, completed within the last two years): COMPASS – Reading 60, ASSET – Reading 35,  or ACT – Reading 13 
  •  College-level English course with a grade of C or better.

If you completed your English course or assessment testing at another college, official transcripts and/or assessment test scores must be sent to the Madison College Enrollment Center. It is your responsibility to verify that the Enrollment Center has received and posted your official English transfer credits/reading assessment test scores, prior to attempting registration.

  •  Final grade of C or better in Academic Reading 5/6
  •  Final grade of C or better in College Reading Strategies.
  •  Official transcripts reflecting completion of a 2-year Technical Diploma, Associates Degree, or higher.

3. Background Check and Health Screening

Note: this health screening process may take up to 3+ weeks, depending on your status of your immunizations.

a. Purchase and create an account on CastleBranch (formerly CertifiedBackground) for your background check and health screening document manager.

The package code for Madison College is MJ17; the cost is $68.00 (subject to change). You do not need to purchase the $12 ID card that is offered. 

b. Print and complete the Background Information Disclosure (BID) Form

c. Print the Health Screening Form and complete page one.

The rest of this form must be completed by a healthcare provider. Madison College does not require a full physical, however some healthcare providers may require a physical before reviewing your immunization records and completing your form.

If you do not have your own healthcare provider the Madison College GHC Clinic is an option.

Note: depending on your vaccination history, you may need updated immunizations or titers (blood tests) to demonstrate your immunity. Tuberculosis (TB) testing may take up to four clinic visits.

d. Scan, save and upload your complete Background Information Disclosure (BID) and Health Screening forms to your CastleBranch account; be sure to include any supporting lab reports or documentation of immunizations.

e. Health or accident insurance is required to take this course. If you do not have your own comprehensive health insurance, you may purchase accident insurance here. The accident insurance is $13 per semester and will be added to your tuition balance.

f. After uploading the required documents, watch your CastleBranch account for any 'to do' messages.

4. Registration

  • When all of the above requirements have been marked complete by Castle Branch, you will receive a message in your Madison College student account notifying you are approved to register.
  • The influenza vaccine is not required, if you are registering for a summer course. If you are receiving a rejection for your Influenza immunization, please contact the School of Health Education at (608)246.6065.

Additional Important Information:

  • Health/accident insurance is required; if you do not have health insurance, you may purchase accident insurance hereThe accident insurance is $13 for the semester and will be added to your tuition balance.
  • Purchase your textbook and Nursing Assistant Supply Kit from the bookstore or regional campus front office no later than the first day of class. There is also a free PDF version of the textbook.  
  • Solid color scrubs and white impermeable shoes are required for lab and clinical portions of the course.
  • Approximate Total Cost: $500-900; a breakdown of this total is available in the Instructions PDF at top of this page.
  • Log into the Blackboard course site about three days before the first day of class to get familiar with the site.
  • If you decide to drop a class, you must drop at least the day before class beings to receive a full refund.
  • Students in a Nursing Assistant course are not allowed to miss any class, lab or clinical.
  • Students must be at least 16 years of age, prior to the first day of class.

Note: Students planning on enrolling in a Nursing Assistant course offered at their high school or through Youth Options have additional steps in this process; please see your school representative for more information.
