- Complete and submit this application. This form must be submitted every semester in which you intend to take classes.
- After college approval, you can register for the course. You will be required to pay tuition and any associated fees at registration.
- At completion of the course you will be required to submit an account/billing statement (indicating tuition has been paid) and a final grade report/transcript (C or better to receive reimbursement). Please provide the information as to who should receive the reimbursement (you or school district).
