The Federal Parent Loan for Undergraduate Students (PLUS) is an optional loan taken out by the parent of a dependent student only. The student must be enrolled in at least 6 financial aid eligible credits.
Completing the PLUS Loan Request Form at Madison College means you have applied online at www.studentloans.gov for the PLUS loan, received a credit decision AND completed a PLUS Loan MPN, if applicable.
STUDENT INSTRUCTIONS - To submit a request for a PLUS Loan, you must first apply for financial aid using the Free Application for Federal Student Aid (FAFSA). Complete the Student Information section below and give to your parent to complete the Parent (Borrower) Information section.
PARENT INSTRUCTIONS - To submit a request for a PLUS Loan, the parent must first complete a Master Promissory Note (MPN) online at studentloans.gov. Then this form must be completed, signed and submitted as described below.
