Graduate Survey Giveway Rules
NO PURCHASE NECESSARY. This giveaway program ("Giveaway") is designed to encourage eligible graduates to complete and return the Graduate Follow-Up Survey or the Apprenticeship Follow-Up Survey. The Giveaway begins at 8 a.m. on Wednesday, January 16, 2019 and ends at 11:59:59 p.m. CT on Sunday, February 2, 2020 (the "Promotion Period"). Only invited Madison College program graduates & apprenticeship completers from the 2018-2019 academic year will be allowed to participate in and are eligible to enter this Giveaway. Each survey that is completed & returned by mail or telephone will count as a single entry in the Giveaway; each survey that is completed online will count as two entries in the Giveaway.
EXCEPTIONS TO THE RULE REGARDING SURVEY SUBMISSION ONLINE:
Due to limitations of the online survey collection system, the following graduates will not be able to complete their survey using the online method:
* Any graduate who completed more than one program at Madison College during a single semester in the 2018-2019 academic year (who will hereafter be called "multiple majors")
These graduates will still be able to complete their surveys by mail or telephone. (A representative from Madison College will make up to two attempts to contact non-responders by telephone during the Promotion Period.) To compensate for the lack of an online submission method, each survey completed by a multiple major will automatically count as two entries towards the Giveaway, regardless of method of submission. For example, if a multiple major graduated from two programs and completed both surveys, it will count as four entries in total.
DEADLINE: To enter the Giveaway, users must (1) complete the survey with all required information, (2) submit the survey by mail, telephone, or online during the Promotion Period. All entries must be submitted and received by 11:59:59 p.m. CT on February 2, 2020. Any survey response received after that date will not be eligible for the drawing. The drawing will be held on or about March 18, 2020. During the "Promotion Period", the Madison College Institutional Research & Effectiveness Department will make several attempts to contact and inform the graduates of the survey and Giveaway.
GIVEAWAY SPONSORS: The Giveaway is being conducted by the Institutional Research & Effectiveness Department of Madison Area Technical College. The grand prize is underwritten by the Institutional Research Support Fund of the Madison College Foundation. At the end of the Promotion Period, one winner will be chosen at random from among all eligible entries received during the Period. The drawing will be conducted by the Madison College Institutional Research & Effectiveness Department, whose decisions are final and binding in all matters relating to this Giveaway.
ELIGIBILITY: This Giveaway is open only to Madison College invited program graduates & apprenticeship completers from the 2018-2019 academic year.
"Madison College graduates" are defined as any person who has satisfied the graduation requirements for any of the following: a 2-year Associate Degree, a 2-year Technical Diploma, the College Transfer program, a 1-year Technical Diploma, and/or a "Less Than One Year" program of study in aid code 10-32. Graduations from a program classified as an Embedded Technical Diploma are included only if the graduate completed an embedded program on a stand-alone basis & is not enrolled in another degree-credit program at Madison College. Any of the following programs are recognized as "Less Than One Year" programs at Madison College: Nursing Assistant, Dental Assistant, EMT-Basic, EMT-Intermediate, EMT-IV Technician, Farm Business & Production Management, Fire Service Certification, Food Service Aide, Law Enforcement Academy, Medical Coding Specialist, Therapeutic Massage Therapy, Custodial Services, and Building Services. The following is a partial list of programs recognized as Embedded Technical Diploma programs: Basic Machining, General Accountancy, IT-Fundamentals of Networking, IT-Fundamentals of Network Security, IT-Fundamentals of Network Security, 3D Arts, Food Service Production, Child Care Services, Industrial Mechanic, and Industrial Mechanic-HVAC.)
"Apprenticeship completers" are defined as a Madison College Apprenticeship student who was awarded their Apprenticeship Completion Certificate by the Wisconsin Department of Workforce Development (DWD) Bureau of Apprenticeship Standards (BAS).
Permanent full-time or part-time employees of Madison College who graduated from an Madison College program are not eligible to participate. Graduates who work at Madison College as interns, "student helpers" or "work study" and who otherwise meet eligibility requirements are eligible to participate.
This promotion is subject to all federal, state and local laws & regulations. Sponsor reserves the right to verify eligibility qualification of the winners and to require winners to sign affidavits of eligibility and liability/publicity releases (where permissible) upon request.
PRIZES/DRAWINGS: A total of 1 prize will be awarded after the end of the Promotion Period.
Prize will consist of:
* One Microsoft Surface tablet.
Odds of winning a prize will depend on the total number of eligible surveys received in the Promotion Period and the method by which they are submitted.
A random drawing by the Madison College Institutional Research & Effectiveness Department to select the potential prize winner will be conducted on or around March 18, 2020 from among all eligible entries received by the deadline. Winner will be notified via email, mail or telephone within one week. Prize will be shipped via the most economical means as deemed by the Madison College Institutional Research & Effectiveness Department. Winner need not be present at time of drawing.
If the prize is returned as undeliverable, or if the selected entrant either cannot be contacted by telephone or email, or does not respond to the affidavit within the requested number of business days allowed in the notification letter, or if selected entrant refuses the prize, the corresponding prize will be forfeited and awarded to an alternate winner selected from all eligible entries received during the Promotion Period. All properly claimed prizes will be awarded.
Taxes on the prize, if any, are the sole responsibility of the winners. By accepting prizes, winners consent to the use of their names/likenesses for publicity purposes worldwide, without limitation or further compensation, except where prohibited by law. No substitution, transfer, or assignment of prize is allowed except as provided herein.
