General Concern / Complaint
This report documents an issue or general concern with a college process, procedure, system, facility, or individual that you are not able to resolve. The concern will be routed to the appropriate department on campus to assist in providing resolution.
Harassment / Discrimination
This report documents incidents or alleged violations of harassment and/or discrimination based on age, ancestry, arrest or conviction record, creed, disability, gender Identity, genetic testing, marital status, military service, race, color, national origin, political affiliation, sex or sexual orientation, or the use or non-use of lawful products during non-working hours. This form can be utilized by anyone wishing to file a report on their own behalf or by anyone wishing to file a report on behalf of an impacted party.
Title IX and Sexual Harassment
This report documents incidents or allegations of sexual violence, sexual harassment, or sex and gender-based discrimination/harassment. This form can be utilized by anyone wishing to file a report on their own behalf or by anyone wishing to file a report on behalf of an impacted party.
Bias Incident Report and Support Form
This report documents alleged incidents of any discriminatory or hurtful act that appears to be motivated by identity characteristics or any other statuses based on a protected class by applicable federal, state, or local law.
